Lessee... Logged in, ported to Tawar Galan, ported to Tanvu, ported to Martok, ported back to Tanvu (forgot to /flush all), crashed -- oh ya and in Martok I also chunked a couple of times.

This one was so similar I'll just tack it on to the end: logged in in Tanvu, did my quest, picked up the next one. Ported to Martok Coast, chunked over to Martok Chunk, then back to the coast. Ported back to Tanvu and crashed at the end of the loading screen.
