This is how I understand it.

Offensive
1) No. It has to come from an Ability as far as I'm aware. And some abilities have an higher innate Crit rate then others. See Leap attack and Kick for example.
2) Correct.
3) Not for warriors, not that i know of anyway. There will be multiple add ons to Decimate however, through Obliterate and Destroy I think it's called. I wouldn't really refer to Decimate as a bridge in that event however.

Defensive
1) Yes.
2) It's similar to offensive, in the sense that you may trigger a Finisher based on a crit from certain abilities.
