Ingot grade determining grade of finished product?

I think I see a pattern in my crafting where the final product often(always) is of the same grade as the ingot used, as opposed to the grade suggested by the crafting process.

I have now noticed several items that in the process were grade A but after finishing came out grade B. These were made from grade B ingots.

I've also had a crafting process that was a D but the final item came out a B and was made from grade B or mainly grade B ingots.

And I've had B's turn into A's while using A ingots.

Anyone else seen the pattern or am I just seeing things....or both?

This is from blacksmithing/weaponsmithing btw. Don't know if it is the same for other professions.
